If a prospective employee from your facility has applied for licensure to a professional board under the New Jersey Division of Consumer Affairs (such as the Board of Physical Therapy, the Board of Medical Examiners, and Board of Nursing etc.) and is experiencing a delay in excess of 6 weeks, please fill out the following form and return to Theresa Edelstein, Vice President, Post-Acute Care Policy at TEdelstein@njha.com.
To help streamline the process by which we raise concerns about the timeliness of an application with the Division of Consumer Affairs, please ensure that your prospective employee has done the following prior to submission of this form:
- The applicant has completed all sections of the application and has included all required documentation and payment and ensured that third-party documents (i.e. transcripts and examination scores) have been forwarded directly to the board.
- It has been at least 10 business days since the applicant was digitally fingerprinted at MorphoTrust or at least 20 business days since the applicant submitted fingerprint cards; and it has been at least 5 business days without communication from the Division.
- The applicant has been forthright about his/her criminal history background. If the applicant has been arrested or convicted of a crime, the applicant must submit a letter of explanation, all relevant police reports, reference letters, and, if applicable, judgments of conviction, sentencing orders, termination of probation orders and evidence of rehabilitation.
